All client forms are kept strictly confidential and are only used to provide you with the safest, most personalized service.

Please answer carefully and truthfully for your aesthetician to determine whether you are suitable and able to undergo the procedure and to rule out any contraindications.

For clients under the age of 18, a parent or legal guardian must sign the Parent/Guardian Consent Form in addition to the standard consent forms signed by the client.
